- A repair receipt should show the shop's name and contact, the vehicle worked on, each part with its price, the labor hours at the shop's hourly rate, a subtotal, the tax on its own line, and the total paid. This template lays those out in that order so the customer can see exactly what they paid for in parts versus labor.

Parts and labor, kept apart
The defining feature of a repair receipt is that it splits parts from labor. A customer paying for a service wants to see what went into the car versus what they paid for the mechanic's time, and warranty and insurance programs require that split too. This template puts every part on its own line and gives labor a line of its own, priced by the hour.
The sample is a routine oil service at a fictional shop, Corner Garage. It shows an oil filter, five quarts of synthetic oil, and an hour and a half of labor — enough to demonstrate the format. Replace the parts and hours with your own job and the structure carries any repair, from a light service to a major fix.
How the sample totals up
Reading the sample top to bottom: the oil filter is $12.00, five quarts of oil at $8.00 each come to $40.00, and 1.5 hours of labor at $110.00 an hour is $165.00. That gives a parts-and-labor subtotal of $217.00. A 7.25% sales tax adds $15.73, for a grand total of $232.73.
Every figure on the receipt follows from the lines above it, which is what makes an itemized repair receipt trustworthy — the customer can check the math. Change any quantity, price or the tax rate and the builder recalculates the subtotal, tax and total for you.
Billing labor by the hour
Labor is where repair receipts differ most from a simple retail receipt. Put the hours in the quantity field and your shop's hourly rate in the price field; the line then reads as hours times rate. The sample bills 1.5 hours at $110, so the labor line is $165.00. If a job has different rates — say diagnostic time and repair time — give each its own labor line so the customer sees how the time was spent.
Some shops bill flat-rate labor from a standard guide rather than actual clock hours. Either works here: enter '1' as the quantity and the flat-rate charge as the price, and describe the operation on the line.
Itemizing the parts
List each part on its own line with a quantity and a unit price, the way the sample separates the filter from the oil. An itemized parts list does three jobs: it shows the customer exactly what was replaced, it ties any parts warranty to specific components, and it builds a service history that adds value if the car is later sold.
For consumables sold by volume, like the five quarts of oil, put the count in the quantity field and the per-unit price in the price field so the line multiplies out. That keeps the receipt honest about how the parts total was reached.
Identifying the vehicle
A repair receipt should say which car the work was done on. The sample records the vehicle in the footer — a 2015 Ford Focus — along with a parts warranty note. Naming the vehicle matters for warranty claims, for a driver keeping records across several cars, and for anyone reconstructing a maintenance history.
The footer is also the place for terms that apply to the whole job: a parts-and-labor warranty period, like the '12 mo / 12k mi' in the sample, or a note that certain work is not covered. Whatever you write there is the promise the customer can hold you to.
For warranty and insurance claims
Because it separates parts from labor, names the vehicle and carries a date and an invoice number, an itemized repair receipt is what warranty administrators and insurers ask for. The sample's invoice number, RO-5590, gives the claim a reference. When a customer needs to submit a repair for reimbursement, hand them the itemized receipt rather than a lump-sum total.
Keep the invoice number visible and the vehicle details filled in, then export a PDF so the customer has a clean, legible copy. A vague receipt can hold up a claim; an itemized one moves it along.
Fill it in and hand it over
Put your shop's name, address and phone in the header, add your parts and labor lines, set your local tax rate, and record how the customer paid. Give the job an invoice number so you can find it again. The totals take care of themselves as you type.
